Job Description

We are seeking an experienced MDS Coordinator for our Skilled Nursing unit. This position involves conducting comprehensive MDS assessments for geriatric patients. The ideal candidate will demonstrate a strong commitment to patient care, ensuring high-quality assessments that adhere to regulatory guidelines.

Responsibilities

• Conducting MDS assessments for geriatric patients.
• Collaborating with interdisciplinary teams to develop and implement care plans.
• Meeting regulatory compliance for documentation and patient records.
• Monitoring and updating patient care assessments as needed.

Required Experience / Certifications / Licensure

• Must have experience as an MDS Coordinator.
• 2 years of experience in MDS assessments preferred.
• Current RN license required.
• Certification in MDS training is mandatory.
